New funding call-out for improving health equity through a population health approach
Grants of up to £250,000 available for projects that create more equitable health outcomes, access and experience for patients at Guy’s and St Thomas’.
We support Guy’s and St Thomas’ to deliver better, faster and fairer healthcare for all.
We are inviting teams from across the Trust to apply for funding to help improve health equity within their services or areas.
This aligns with the Trust’s 2030 Strategy and its priority: Improving the health of our populations – by helping people to live longer, healthier lives, and narrow the gap in life expectancy between the most and least advantaged populations.
This call-out will be supported by the Trust’s newly established Population Health Hub (PHH). The Hub was created with thanks to charity funding and aims to improve population health and reduce health inequity.
What is health equity?
Health equity is the state in which everyone has a fair and just opportunity to attain their highest level of health and no one is disadvantaged from achieving it. In a hospital setting this means designing and delivering care where access, quality, experience and outcomes do not vary by patient characteristics.
What is a population health approach?
A population health approach considers the health outcomes of a specific group of individuals. It is informed by data and is evidence based. Population health is considered within the context of an integrated health and care system, the wider determinants of health, health behaviours and lifestyles, and the communities we live in.
